Vassall Medical Centre is a practice that offers a high quality of primary care services for a diverse population, with focus on inclusion and equality.Our size list is around 7800 patients and our clinical team consists of 2 GP Partners, 2 Salaried GPs, 1 practice nurse, 1 HCA, 2 pharmacists. Our supportive admin team includes an IT manager, 1 practice manager, 1 Managing Partner and a great team of administration / reception staff.Hours of work will be between 7 am- 6:30 pm, excludes weekendsJob responsibilities
Professional ResponsibilitiesAssistpractice GPs/NP to deliver effective and holistic management of patients withchronic diseases as required by the Quality and Outcomes framework.Contribute to the development of policies / protocols / guidelines within thepractice.Aidthe practice team with implementation of new initiatives in the delivery ofchronic disease management/enhanced services.Formulate reports as required for the practice manager and or nursepractitioner.Support the implementation of evidence-based guidelines in conjunction with thepractice clinical team.Encourage the development of integrated services within Primary Care to includethe practice based teams and multidisciplinary working with external providers.Promotethe continuing professional development and skills of both yourself and othernurses within the practice.LeadershipProvide professional advice and support to the practice nursing teams.Offerguidance and support to the practice nursing teams to maintain/improvestandards of care.Participate in chronic disease management within the practice and providepractical assistance and professional advice to achieve the highest standardsof care.Provide clinical input where required alongside other practice nurses,particularly newly appointed nurses, to assist in skill development and sharingof best practice.Delegate to others, with appropriate skills, knowledge and competence.Maintain nursing practice in line with relevant evidence based research.Act asa resource to the general public, patients within the health care system, theirfamilies, carers and external healthcare professionals.Infection Control leadClinicalWorkalongside other practice staff to help develop their knowledge and skills.Support the practice in achieving 100% QOF and enhanced service targets.Undertake opportunistic screening when clinically required/indicated.Perform venepuncture and electrocardiography where required.Undertake wound care/ management/dressing techniques.Carryout ear examination and syringing.Undertake family planning assessments, provide advice and refer patientsappropriately.Perform cervical screening and promote breast awareness.Administer adult and paediatric injections under an individualised prescriptionor Patient Group Direction, ensuring safe storage, rotation and disposal ofvaccines and drugs.Undertake accredited training/updates in chronic disease management.Contributeto the maintenance of the practice disease registers.Provide advice on lifestyle issues, assist and support patients in changingtheir behaviours and refer appropriately.Support the adult and childhood immunisation service working to Patient GroupDirections. Deliver travel healthcare, including vaccinations and advice forpatients.Recognise the signs of child abuse, family violence, vulnerable adults,substance abuse, addictive behaviours, suicidal tendencies and demonstrateawareness of local referral procedures.Support the implementation and effective use of the child safe trigger toolApply infection control measures when collecting and handling laboratoryspecimens, disposing of waste materials, dealing with blood and body fluidspillages and report and treat sharps injuries according to protocol.Undertake annual training and demonstrate competency in anaphylaxis andresuscitation techniquesResearch, Audit and QualitySupport the practice audit strategy, action plan and evaluation.Contribute to the provision of equitable, high quality services for patients inthe locality.Helpsupport the integration across primary, secondary and tertiary care services.Maintain an interest in national research initiatives (NICE) and disseminateappropriate information to health professionals as required.Ensure all care is evidence based using up to date validated literature.Education and TrainingProvide professional advice/support to healthcare professionals within thePrimary health care teamFollowing identified training needs, participate in training, facilitation andclinical supervision of other practice nurses.Provide mentorship and guidance for junior staff.Participate in partnership programmes, raising awareness, detection andprevention of chronic diseases.Maintain a high level of expertise in specific chronic disease management,specifically Diabetes, COPD and Asthma and seek to develop new and improvedskills.Advise members of the public on the concepts of health promotion, education,prevention and protectionSeekout new knowledge of nursing and health by reading, enquiring and partaking ininternal and external continuous education programmes.Takeresponsibility for personal development and education including in-housetraining and monthly PLT.Actively participate in multidisciplinary education and training.Direct, supervise and help mentor junior nursing staff in the clinical setting.Monitor and maintain consistency of professional standards and take appropriateaction if standards are not met.Maintain professional development by attending relevant courses and study daysto keep abreast of current knowledge and developments.Abide by the legal requirements and statutory rules relating to practice andmaintain periodic professional registration with the NMCParticipate in an annual appraisalPerson Specification
