We are looking for an enthusiastic Practice Nurse to join our multi-disciplinary team to provide a range of services which includes Chronic Disease Management, Cytology, Immunisations, Asthma, Diabetes Management and all tasks related to being a Practice Nurse.You would need to have experience of working in accordance with the UKCC Code of Professional Conduct and The Scope of Professional Practice. Experience of EMIS WEB clinical system is an advantage.You would need to possess excellent communication skills and can demonstrate active involvement in a general practice setting.The successful candidate for this role will enjoy working as part of a dedicated team who will receive an excellent basic salary commensurate with experience and with further development opportunities and an opportunity to join NHS Pension Scheme.We expect our nursing team to contribute to the growth and maintenance of high standards within our organisation.Main duties of the job

The main duties of the role is chronic disease management clinics i.e. diabetes, asthma, COPD and hypertension) Carrying out health checks and screening (cytology etc), administering immunisations and vaccinations, wound care management, travel immunisations, and any tasks related to being a Practice Nurse.The Practice Nurse will work as part of the primary healthcare team providing Personal Medical Services to the patients of the Practice.To be involved in Health promotion, disease management, QOF and audit programmes in accordance with practice protocols and policies.About us

We are also supported by a team of Allied Healthcare Professionals, i.e. Social Prescriber, Health Coach, Mental Health Practitioner and First Contact Practitioners. The practice is well organised with an excellent team of admin and receptionists. Qualified Practice NurseREPORTS TO:THE PARTNERS (Clinically)THE PRACTICE MANAGER (Administratively)Grade: F/GHOURS: PTJob Responsibilities:Assist in and perform routine tasks related to patient care as directed by GPs. To provide essential Treatment duties such as BP checks, NHS Health checks, new patient medicals/urinalysis, prescribed injections (e.g. B12 and Denosumab) BMI and lifestyle adviceCytologyTo provide holistic assessment of wounds and implement wound care in line with current evidence-based guidelines. Inclusive of the treatment of minor traumatic wounds within own level of competency, ulcer care/Doppler,Removal of SuturesTravel ClinicECGsAsthma/Peak Flow Readings/SpirometryVenepunctureEar syringingOffer advice about childhood and adult immunisations and ensure vaccines/injections are administrated under patient group directives or individual prescription.Provide contraception advice and sexual health guidance. Undertaking annual contraceptive reviews and injections in line with practice policies.Assisting GPs with minor surgery and coil fittings, implantsRequesting pathology tests, for example urine culture, swabsInterest in long term conditions i.e. diabetes, coronary heart disease, COPD etc., following agreed clinical protocols with referral to GPs as appropriate.To be able to recognise and manage anaphylaxis, according to the current UK Resuscitation Council guidelines.To be able to perform Cardio-pulmonary resuscitation and the correct use of Automated Defibrillator following training, according to the current UK Resuscitation Council guidelines.To run the Anticoagulant clinic to Practice patients*(*See Anticoagulant service specification) Training will be provided.Work in accordance with NMC The Code for Nurses and Midwives (2015).Ensure accurate notes of all consultations and treatments are recorded in the patients notes and on computers, as per NMC guidelines.Other tasks will include: -To maintain effective infection control measures in line with local guidelines and practice policy.Ensure safe storage, rotation and disposal of vaccines and medication.Assist in the completion of monthly stock take and support the cost-effective use of disposable items.Chaperoning and assisting patients who are being examined by another clinician.Maintain general tidiness and cleanliness of nurses and treatment rooms.Ordering of Health Promotion LiteratureMaintain stock of health promotion literatureOrdering of stock from the HospitalOrdering of vaccinations to maintain stock levelsParticipation in administrative systems in the PracticeAt all times there will be a need to maintain accurate recordsA duty to advise GPs of potential problems or errors within the range of assigned tasks.Attend and participate in weekly clinical meetings and any Practice meetings when required.Awareness of statutory protection procedures including referral and recognition of signs and symptoms of child abuse.Awareness of referral mechanisms for mental health issues, family violence, vulnerable adults, and addictive behaviours.Actively take part in mandatory and internalexternal training as requested.Adhere to and follow all policies and protocols as listed in the Employee Handbook and contract.On occasion there may be a requirement to undertake home visitsAny other delegated duties appropriate to the postConfidentiality:In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.
