The ideal candidate will have experience in the core competencies of practice nursing including wound care, child immunisation, independent and proactive chronic disease management (in particular diabeties) and cervical cytology. In addition, experience of management responsibilities would be beneficial, as there will be scope for the successful applicant to take on additional individual organisational and management responsibilities.About us

Job summary:Helping the practice develop and deliver specialist primarynurse led services including chronic disease management. Make professionallyautonomous decisions and provide safe, evidence based, cost effective,individualised patient careProviding assessment, screening and treatment services andhealth education adviceCytologyWound careTravel ClinicAsthma/Peak Flow ReadingsRoutine immunizations/Childhood immunisationsRequesting pathology tests, for example urine culture, swabsDiabetic reviewsOther tasks will include:Maintain general tidiness and cleanliness of nurses andtreatment roomsMaintain stock of waiting room leaflets as instructed bypartners and Practice ManagerOrdering of vaccinations to maintain stock levelsAttend and participate in any Practice meetings whenrequired.Following appropriate training, undertake the collection,storage and despatch of pathological specimens including intravenous bloodsamples, swabs, smears etc. and ensure the cleanliness of any equipment soused. Perform any investigatory procedures requested by the GPs.Ensure accurate notes of all consultations and treatmentsare recorded in the patients notes on the clinical computer system asappropriateEnsure accurate completion of all necessary documentationassociated with patient health care and registration with the practiceAttend and participate in practice meetings as requiredMaintain a notice board in the waiting areadesignated for patient health care educationPerson Specification
Qualifications
Qualified NurseQualified in management of diabetesExperience
