Community Staff Nurse Band 5 - Congleton and Holmes Chapel

Overview
The Community Nursing Team works as part of a wider multi-disciplinary team, caring for residents in Congleton and Holmes Chapel. We work in partnership with practice nurses and empower patients to deliver the lower leg wound pathway. We enable patients to manage their long-term conditions whilst remaining at home and preventing unnecessary admissions to hospital. Our work enables us to integrate with other services in the community including virtual wards and crisis response. Congleton War Memorial Hospital is currently undergoing exciting changes and opportunities to link with the development of the community diagnostic centre.Hours per week: 30 hours.
Responsibilities
Provide patient-centred care within the community nursing team and multi-disciplinary teams.Work across the Congleton and Holmes Chapel patch and contribute to ongoing service development.Support patients to manage long-term conditions at home and prevent unnecessary hospital admissions.Collaborate with practice nurses and other community services, including virtual wards and crisis response, to deliver integrated care.Assist in the advancement of the team and participate in service improvement initiatives.
Qualifications and Requirements
Excellent, up-to-date clinical skills; enthusiastic, motivated and able to work flexibly across the patch.Experience is not necessary; enquiries are welcome from those interested.DBS check required (Enhanced with Barred List); overseas police clearance may be needed if you have lived or worked abroad in the last 5 years.Please note: we are unable to provide Skilled Worker Visa Sponsorship to non-UK residents.
