Overview

Job Title: Community Learning Disability Nurse / RMNBand 6: Full Time: 37.5Base: Community Learning Disability Service, Mile End Hospital, London E1 4DGThe Community Learning Disability Service provides integrated specialist health and social care to adults with learning disabilities in Tower Hamlets. The service also oversees a specialist day activity project – CREATE.Responsibilities

Use nursing skills in working with people with learning disabilities and autism to maintain, promote and improve health.Assess and support people with learning disabilities and complex health needs, autism and commonly coexisting conditions such as ADHD and mental illnesses.Fully involve the person with a learning disability who is at risk of behaviours that challenge.Support people with learning disabilities to develop their full potential by reducing the impact of their impairment that could limit social inclusion.Facilitate access to health services through collaboration and partnership with primary and secondary health, and other relevant services.Use a person-centred approach to promote choice, independence and inclusion.Contribute to shaping local health services to promote an inclusive service for people with learning disabilities.Undertake assessments, reviews and care planning for NHS Continuing Healthcare Funding in accordance with the National Framework for NHS Continuing Healthcare.Understand the implications of the legal and policy framework underpinning the support of people with a learning disability and autism.Key Working Relationships

The post holder is line-managed by the Care Team Manager (Integrated Health and Social Care) – Integrated Multi-professional Team. The post holder is professionally accountable to the Clinical Nurse Specialist (Lead Nurse).Maintain multi-professional working relationships within CLDS, Primary and secondary care services and other agencies that support people with learning disabilities in Tower Hamlets.Requirements

You will be expected to be RNLD or RGN qualified.The role involves working with individuals with learning disabilities and autism, including those with complex health needs.Criminal Records Disclosure: The post falls within the Criminal Records Disclosure Policy and therefore a criminal record check will be applied for the successful candidate.Additional Information
