This role is pivotal in supporting veterans at risk of homelessness and aims to make a significant impact in the community.Key Responsibilities: Act as the first point of contact for inquiries, efficiently managing the referral process.Conduct initial assessments of veterans'' housing status and identify root causes of their insecurity.Collaborate with various stakeholders to facilitate access to appropriate accommodation and support services.Maintain accurate records of all referral and case management activities, ensuring compliance with contractual requirements.Use trauma-informed approaches to empower veterans, guiding them towards positive choices and support pathways.
