Band 6 Specialist Physiotherapist

Job summary We have an exciting opportunity for a Specialist Physiotherapist at Buckinghamshire Healthcare NHS Trust to join the Rapid Response and Intermediate Care (RRIC) Teams across Thame and Buckingham. In this community-based role, you will be responsible for supporting patients with complex health needs through personalised rehabilitation and case management approaches. You will work closely with Advanced Community Practitioners and multidisciplinary colleagues to ensure timely, effective care delivery, while supporting workload coordination across the service. You will utilise your specialist clinical expertise to assess patient needs, deliver evidence-based interventions, and help identify individuals at risk of hospital admission, ensuring they receive appropriate support to achieve the best possible health outcomes. Main duties of the job As a Specialist Physiotherapist, you will be responsible for contributing to the development and implementation of clinical pathways, ensuring service delivery is aligned with agreed standards, protocols, and evidence-based practice. You will support the creation and ongoing review of operational procedures and guidelines within the RRIC service to enhance consistency and quality of care delivery. You will be responsible for supporting service resilience by participating in 7-day working rotas, including weekend cover and Urgent Community Response duties across Buckinghamshire localities.
