This is a long-term opportunity with ongoing block bookings available for the right candidates.Shift Patterns• Monday - Friday: 08:00 - 17:30• Late shifts: 13:00 - 21:00Key Responsibilities• Manage a community caseload independently • Deliver high-quality patient care in community settings • Conduct patient assessments and care planning • Provide palliative and chronic disease management • Work collaboratively within MDT environments • Ensure accurate documentation and care continuityEssential Requirements • NMC Registered General Nurse (RGN) • Proven Band 6 District/Community Nursing experience • Strong background in palliative care and long-term conditions • Competent in wound care, catheter care and medication administration • Ability to work autonomously and manage workload effectively • Full UK driving licence and access to a vehicle (essential) • Up-to-date mandatory training • Valid DBSAdditional Information • Ongoing work availableHow to Apply Please submit your CV on the below email: ..... full job details .....
