The CherwellHospital is currently looking to recruit anexceptional Pre-Assessment Nurse to provide specialist nursingexpertise, facilitating evidence-based patient-cantered care andassessment for a defined caseload of pre-assessmentpatients.To work both independentlyand collaboratively with a team of nurses and healthcareassistants, enabling them to co-ordinate patients throughout theirpre-assessment pathway, with a high level of input into themanagement and care of patients prior to admission for specifiedprocedures.To act as a resource and to provideeducation for patients and health care personnel, and to direct thepre-assessment nursing team.Carry out relevantforms of care without direct supervision, working effectivelywithin the multi-disciplinary teamTo supportof all members of staff within the Outpatient department, includingstudents of nursing/visiting healthcareprofessionalsActively participate in theteaching and mentoring of students and juniorstaffTo suggest improvements in careprovision, recognizing and challenging poor practice, raisingconcerns to the relevant agencies whereappropriate.Ensure that the nursing philosophyof care for Pre-operative assessment is reflected inpractice.To ensure provision of a highstandard of nursing care to patients attending the Outpatientdepartment working within agreed policies andprocedures.To act as a liaison link with theMulti-disciplinary team and contribute to work involved in theAnesthetic Meeting / Complex MDT meeting - for example - escalatingcomplex patient and help plan theircareCollaborates and consults with themulti-professional health care team and the patient, communicatingvery sensitive, complex information.Useshighly developed communication skills to provide guidance, adviceand support to the patient / ward / physio / theatre staffregarding management of their proposed admission and its impact onpatient’s/carer’s life.Whilst communicatingcomplex, sensitive, sometimes distressing information to patient,is able to identify and overcome barriers to patientunderstandingProvides verbal and writteninformation to patient, to enable them to give informed consent fortreatment, using specialistknowledge.Establishes partnerships withpatients/carers and communicates with them, sometimes in highlydistressing or emotionalcircumstancesProvides, receives, interpretsand communicates complex information with a range of health carepersonnelAct as patients advocate, ensuringthat informed consent, privacy and dignity are maintained(NMC)Carry out health promotion within jobrole utilising information leaflets and networking with otherspecialist healthcare staff around the Trust wherenecessaryCritically reflect on own performancethrough supervision/actionlearning
