Social Worker Adults

We are currently recruiting for an experienced Social Worker to join the Transitions Team in Halton. The team supports young people aged 14 and above who meet Care Act eligibility criteria, helping them move from children''s to adult services with the right support in place.In this role, you will be responsible for screening referrals and completing Care Act assessments, ensuring young people receive appropriate, person-centred care and support plans.Key Responsibilities:Undertake screening of referrals into the Transitions TeamComplete Care Act assessments in line with statutory requirementsWork collaboratively with young people, families, and multi-agency professionalsSupport effective transition planning from children''s to adult servicesMaintain accurate and up-to-date case recordsRequirements:Qualified Social Worker with relevant registrationProven experience completing Care Act assessmentsStrong assessment and analytical skillsAbility to work effectively with families and multi-disciplinary teamsDesirable:Previous experience working with Looked After Children (LAC) - preferred, not essential Experience within a Transitions or Disability TeamEssential Skills and Knowledge: A strong understanding of how disability impacts family life is essential, along with the ability to work sensitively and effectively with young people and their families during key transition stages.Why Join Us?Supportive team environmentOpportunity to make a real impact in young people''s ..... full job details .....
