Social Worker Adults - Complex needs

We are currently recruiting for an experienced Social Worker to join the Complex Care Team in Runcorn to support with a service backlog and ongoing case management.This is an excellent opportunity for a qualified and confident practitioner who has successfully completed their ASYE programme and has experience working within adult social care and complex case management.Key ResponsibilitiesCompleting Care Act AssessmentsUndertaking outcome-focused reviewsCarrying out Mental Capacity Assessments and Best Interest DecisionsSupporting with safeguarding enquiries under Section 42Completing CHC checklists and attending DST meetingsParticipating in duty rota responsibilitiesWorking collaboratively within a multi-disciplinary team (MDT) approach alongside internal services and external partner agenciesRequirementsQualified Social Worker with Social Work England registrationCompletion of ASYE programmePrevious experience within Adult Social Care / Complex CareStrong knowledge of the Care Act, Mental Capacity Act, and safeguarding legislationAbility to work independently and manage a varied caseloadConnect2Halton is a trading style of Halton and Kent Commercial Services LLP- A joint venture between Halton Borough Council and Commercial Services Kent Ltd.
