Social Worker Adults

We are currently recruiting for an experienced Social Worker to join the Transitions Team in Halton. The team supports young people aged 14 and above who meet Care Act eligibility criteria, helping them move from children''s to adult services with the right support in place.

In this role, you will be responsible for screening referrals and completing Care Act assessments, ensuring young people receive appropriate, person-centred care and support plans.

Key Responsibilities:

• Undertake screening of referrals into the Transitions Team
• Complete Care Act assessments in line with statutory requirements
• Work collaboratively with young people, families, and multi-agency professionals
• Support effective transition planning from children''s to adult services
• Maintain accurate and up-to-date case records

Requirements:

• Qualified Social Worker with relevant registration
• Proven experience completing Care Act assessments
• Strong assessment and analytical skills
• Ability to work effectively with families and multi-disciplinary teams

Desirable:

• Previous experience working with Looked After Children (LAC) - preferred, not essential
• Experience within a Transitions or Disability Team

Essential Skills & Knowledge:
A strong understanding of how disability impacts family life is essential, along with the ability to work sensitively and effectively with young people and their families during key transition stages.
