Overview
Social Care Practitioner (Occupational Therapy) – Derbyshire County Council. Permanent 18.5-hour contract based in Clowne, covering the North of the County.Responsibilities
Deliver a proportionate, personalised, safe and sustainable response for local adults, their carers and families, using a strength-based approach.Work as part of a multi-disciplinary Adult Care Team, collaborating with Occupational Therapists, Team Coordinators, a dedicated OT service manager and senior practitioners.Contribute to inter-agency and multi-disciplinary approaches across the council and other statutory partners.Assess activities of daily living and adapt support to enable maximum independence.Manage and prioritise casework effectively.Travel for home visits as required.Demonstrate risk awareness and personal resilience within everyday practice.What you’ll bring
Knowledge and/or experience of working in a person-centred, holistic and strengths-based way.Knowledge and/or experience in assessing a person’s activities of daily living.Experience with physical, learning disabilities and/or mental health conditions and their impact on daily living.Knowledge of equipment and adaptation solutions.Ability to manage and prioritise casework.Good communication skills and the ability to form positive relationships with partners, professionals and agencies.Good IT skills, time management, and high motivation.Understanding of statutory legislation underpinning Occupational Therapy assessment and intervention in adult social care and health.Reflective practitioner with a commitment to supervision and professional development.Knowledge and/or experience of safeguarding principles and their everyday application.What we offer
