Senior Primary Care Mental Health Nurse

Overview
This Post is a permanent position working in the Maldon/Braintree area for Primary Care Mental Health service in Essex. The post will be based in General Practice, working with existing Primary Care teams to triage, assess, manage and follow up patients and provide brief intervention to those registered with GPs in the area. You must be highly skilled and motivated, work autonomously and be confident in providing evidence based psychological and non-psychological treatments for common mental health disorders following your assessments. Excellent skills of assessment and knowledge of local services are essential, as the practitioner will be the gate keeper and guardian for referral to secondary services. The practitioner will work in collaboration with Primary Care colleagues, providing support, specialist advice, consultation, education and problem solving to Primary Care colleagues for the assessment and management of service users with common mental health disorders. You will act as a sign poster for other internal and external services where required and be at the forefront of developing integrated care pathways.
Main duties of the jobExperience of working within a Primary Care Mental Health setting as a qualified Mental Health Nurse
Ability to work under pressure and manage the stress of others and oneself
Robust clinical judgement and the ability to work autonomously with appropriate level of responsibility
Participate in patient safety investigations, complaints and relevant investigations in line with Trust policies
Mentorship and supervision of students
Develop innovative practice to address service demands for referrals, allocations, monitoring of KPIs and clinical caseload management
Hold a UK driving licence; travel to Primary care network bases in Braintree/Maldon and trust sites as required
Work with GP, Primary Care team and Consultant Psychiatrists to ensure patients are supported in the most appropriate clinical practice environment from a multi-disciplinary perspective
Triage and assess patients to facilitate the most appropriate support and avoid unnecessary transitions through the system
Manage and deliver effective and safe services aligned with clinical/practice governance
Provide and undertake detailed physical health investigations such as ECGs with basic interpretation
Non medical prescriber is preferred
Knowledge of SystmOne patient documentation system

Job responsibilities
Create a single path within the sphere of responsibility and work closely with other Community Mental Health teams to provide a seamless service in the community to support patients’ recovery and wellbeing plans. Work with the GP, Primary Care team and Consultant Psychiatrists to ensure patients are supported in the most appropriate clinical environment from a multi-disciplinary perspective. Ensure safe clinical practice and maintain Care Quality Commission standards in line with the NHS Constitution. Triage patients to facilitate appropriate support and avoid unnecessary handoffs. Manage and deliver effective and safe services aligned with governance. Lead teams as the Primary Care model develops to continuously improve quality and safeguarding standards. Provide and undertake physical health investigations (ECGs) and interpretation. Non medical prescriber preferred.
Professional practiceTrained Advanced Nurse Practitioners (Independent Non-medical prescribers) with diagnostic skills may provide consultation, diagnosis and prescribing for specific service users
Supervision and management of lower banded staff within the professional group and team locality as required
Establish strong working relationships within the primary care setting
Provide educational support and training to enhance knowledge of mental health and wellbeing
Provide professional clinical leadership at multi-disciplinary meetings
Adhere to Trust policies and contribute to new policy development
Maintain professional accountability with supervision and dissemination of best practice
Contribute to Trust and government directives; ensure delivery of high-quality care and treatment
Collaborate with mental health professionals, statutory and voluntary bodies; manage risks and complete risk assessments
Person specificationEducation and Qualifications
Essential: Registered Mental Health Nurse (RMN)
Essential: Non-Medical Prescriber
Desirable: Venepuncture/ECG trained; physical health screening
Knowledge
Essential: risk management / suicide prevention training; mentorship training; good written and spoken English
Desirable: experience using SystemOne
Skills and Experience
Essential: experience in a primary care setting; IT/computer skills; good English communication
Desirable: experience using SystemOne
Personal Qualities
Essential: ability to work under pressure; self-motivated; works well in a team
Desirable: ability to manage priorities of workload
Corporate
