Overview
This post is a permanent position working in the Maldon/Braintree area for Primary Care Mental Health service in Essex. The post will be based in General Practice, working with existing Primary Care teams to triage, assess, manage and follow up patients and provide brief intervention to those registered with GPs in the area. You must be highly skilled and motivated, work autonomously and be confident in your ability to provide evidence-based psychological and non-psychological treatments for common mental health disorders following your assessments. Excellent skills of assessment and knowledge of local services are essential to this role, as the practitioner will be the gatekeeper and guardian for referral to secondary services. Working in collaboration with Primary Care colleagues, the practitioner will provide support and specialist advice, consultation, education and problem-solving skills to Primary Care colleagues with regards to the assessment and management of service users with common Mental Health disorders. You will act as a signpost for other internal and external services appropriate to service users\'' needs, where required and be at the forefront of developing integrated care pathways.Responsibilities
Work in conjunction with the GP, Primary Care team and Consultant Psychiatrists to ensure patients are supported in the most appropriate clinical practice environment from a multidisciplinary perspective.Triage patients and facilitate them receiving the most appropriate support for their needs, ensuring people are not passed around the system.Manage and deliver effective and safe services which support the key elements of Clinical/Practice governance.Provide and undertake detailed Physical Health investigations such as ECGs and offer basic interpretation of the results (Non-medical prescriber preferred).Establish strong working relationships within the Primary Care setting and provide educational support and training to enhance knowledge of Mental Health and Wellbeing.Provide professional clinical leadership at multi-disciplinary meetings (referral/review, safeguarding, etc.).Adhere to Trust Policies and Procedures and contribute to ongoing review and implementation of new policies, practices and procedures.Ensure professional accountability by liaising with the Trust Physical Health Lead and ensuring Clinical and Professional supervision is undertaken.Contribute to development and implementation of Trust and Government directives within the locality and ensure delivery of high-quality care.Work with statutory and voluntary organisations and community networks to identify, assess and manage risks, including completing risk assessments and implementing risk treatment plans.Provide training where appropriate for other professionals; deliver timely statistics to the Information Department and monitor KPIs.Be involved in medication administration (nurse applicants) and provide education on medications, side effects and adherence; support to develop service users\'' understanding of physical healthcare.Lead and participate in formulation, delivery and review of individualized evidence-based care packages according to NICE guidance, including non-medication strategies.Develop and maintain relationships with statutory/non-statutory agencies involved in physical and mental healthcare.Assist the Team Manager with policy development and operational/clinical policy implementation.Deliver training for other professionals and ensure high-quality care and treatment within the clinical setting to service users and carers.Complete and return timely, accurate statistics; ensure compliance with outcomes/KPIs; identify improvements in conjunction with the MDT and management.Provide information and education to patients and relatives to promote dignity, independence and quality of life.Qualifications and Requirements
Trained Advanced Nurse Practitioners (Independent Non-medical prescribers) with diagnostic skills who can provide consultation, diagnosis and prescribe for a specific group of service users.Experience of working within a Primary Care Mental Health setting as a qualified Mental Health Nurse.Ability to work under pressure, manage stress for self and others, robust clinical judgement and ability to work autonomously.Mentorship and supervision experience of students.UK driving licence essential to drive to Primary Care network bases in Braintree/Maldon and Trust sites; some areas may be remote with limited public transport.Benefits
