Senior Case Worker (Smoking Cessation)

As a Senior Case Worker, you will support adults with care and support needs to improve their health, wellbeing and independence through strengths-based reviews and personalised smoking cessation support. Working with people who may face barriers to accessing mainstream services, including those with disabilities, long-term conditions, mental health needs and complex social circumstances, you''ll build trusted relationships and empower individuals to make positive, lasting changes that support a healthier future. Working within the Younger Adults Team, you''ll collaborate with Social Workers, Public Health colleagues and partner organisations to embed smoking cessation into social care practice. Through compassionate, non-judgemental conversations, tailored interventions and effective partnership working, you''ll help individuals understand the impact of smoking on their health and wellbeing, access stop smoking services and wider community support, and contribute to the ambitions of Smokefree Hampshire 2030. What you''ll do Complete assessments, reviews and community-based visits, identifying opportunities to support people to improve their health, wellbeing and independence through smoking cessation. Use strengths-based and motivational approaches to help people overcome barriers to quitting smoking and achieve positive outcomes.
