Reablement Assistant Purpose of the Role To provide practical, personal and emotional support to individuals in line with their person-centred support plans, enabling them to maintain independence, develop daily living skills and remain safely within their own homes and communities. Key Responsibilities Support service users with agreed tasks and outcomes identified in their individual support plans. Provide personal care, including the safe use of moving and handling equipment in accordance with training and best practice. Assist individuals to develop, maintain and improve daily living skills, using appropriate aids and equipment where required. Work collaboratively with other professionals and services to reduce risks and support individuals to live safely and independently in their own homes. Encourage and support individuals to maximise their independence in areas such as meal preparation, kitchen skills, household tasks, laundry, and personal health care within a safe and supportive environment. Work as part of an integrated team with allied health professionals and social care colleagues to provide coordinated support. Support the implementation of person-centred support plans and contribute to assessments and review processes when required. Promote high standards of care practice, ensuring individuals are treated with dignity and respect and that their independence, cultural, spiritual and religious needs are recognised and valued.
