This role is crucial in supporting young people aged 16-25 who have been or are looked after by the council, assisting them in their transition to adulthood and independent living.Day-to-day of the role:Manage a caseload of young people, maintaining accurate case records and undertaking statutory recordings.Contribute to and provide ongoing assessment for Pathway Plan Assessments, promoting full engagement of young people.Undertake assessments for young people resuming or wishing to resume education/training or those presenting with other support needs post 21 years.Work closely with young people to promote their welfare and ensure their views and wishes are considered in assessments, Pathway Plans, and reviews.Provide flexible advice, practical assistance, and support to enable successful transitions for young people.Coordinate the provision of services and ensure young people make use of such services.Identify and supervise the delivery of support packages for young people living in the community, in accordance with their pathway plan.Ensure young people receive appropriate financial support and assist them in developing financial management skills.Support young people in gaining access to appropriate accommodation.Work in partnership with a range of agencies to ensure an integrated, holistic ..... full job details .....
