This role is ideal for someone passionate about making a significant impact in the lives of vulnerable residents, ensuring they are prepared and supported to live independently.Day-to-day of the role:Manage a caseload and maintain accurate case records, undertaking statutory recordings.Contribute to Pathway Plan Assessment and provide ongoing assessment to inform Pathway Planning.Undertake assessments for young people resuming education/training or presenting with other support needs post 21 years.Promote welfare and ensure the views and wishes of young people are considered in assessments, Pathway Plans, and reviews.Provide advice, practical assistance, and support to young people to enable successful transitions.Coordinate the provision of services and ensure young people make use of such services.Identify and supervise the delivery of support packages for young people living in the community.Ensure young people receive appropriate financial support and help them develop financial management skills.Advise and support young people in gaining access to appropriate accommodation.Work in partnership with a range of agencies to ensure an integrated, holistic response to meet the needs of young people.Required Skills ..... full job details .....
