Develop personalised care plans to meet the unique needs of each child.Oversee and support a team of carers, ensuring high clinical standards.Provide ongoing monitoring, assessments, and interventions.Collaborate with multidisciplinary teams, CHC nurses, and CCG commissioners to maintain bespoke care packages.Build strong relationships with children and their families.Package and Benefits

Annual salary between £45,000 - £50,000.45p per mile for business travel.Hybrid working arrangement - 2 days from home, 3 in the community.Monday to Friday, 9-5 schedule with no weekends or nights.33 days holiday.Private pension scheme.Excellent learning and development programme.Company mobile phone and laptop provided.The ideal candidate
