Mental Wellbeing Practitioner Location: Chelmsford Salary: £25,000 per annum Vacancy Type: Permanent About The Role Are you passionate about supporting people to improve their mental wellbeing and overcome complex challenges? We are looking for a dedicated and compassionate Mental Wellbeing Practitioner to join our team at HMP Chelmsford. In this rewarding role, you will work directly with adults experiencing co-existing mental health and substance misuse issues, providing tailored support through both one-to-one and group interventions. You will play a key role in helping individuals improve their wellbeing while working as part of a multi-disciplinary team within the prison environment. Key Responsibilities Manage a caseload of individuals with complex mental health and substance misuse needs. Deliver individual and group-based mental wellbeing interventions. Undertake assessments, develop care plans, provide advice and guidance, and make referrals to appropriate services where required. Build positive and effective working relationships with partner agencies to ensure coordinated support for service users. Attend multi-agency meetings, representing Forward Trust and providing updates on service delivery and outcomes. Contribute to achieving organisational and commissioner targets, with a strong focus on measuring positive outcomes for those accessing the service.
