Overview

Croydon Health Services NHS Trust is seeking a dedicated and organised pharmacist to lead and support the High Cost Drug (HCD), Medicines Information (MI) and Formulary team. The successful applicant will have previous experience in HCD, MI and Formulary. Our Pharmacy Services work closely with multi-disciplinary teams to improve patient care. The Trust has a history of adopting electronic prescribing and administration for inpatients and outpatients and continues to develop the system.Responsibilities

Lead the day-to-day activities relating to High Cost Drug (HCD), Medicines Information (MI) and Formulary to high standards and in line with trust values.Engage with stakeholders both within the Trust and from external organisations to ensure seamless patient care.Line manage other staff and support education and training of team members.Implement objectives and strategies set out for HCD, MI and Formulary.Collaborate closely with the Principal Pharmacist for Medicines Management and other colleagues to ensure all tasks related to Medicines Information, Formulary, High Cost Drugs and Homecare are carried out.Participate in the pharmacy late-duty, on-call and weekend work rotas.About Croydon Health Services
