Job Title: Rough Sleeper Prevention Worker Salary: 25,235 per annum Hours: 37.5 hours per week Contract Type: Permanent Location: ExeterAdditional Information:A valid UK driving licence and access to a vehicle (business insurance required)Participation in an outreach rota, which may include early mornings, evenings, and weekendsStandard working pattern: Tuesday to SaturdayAbout the RoleOur client is seeking a dedicated Rough Sleeper Prevention Worker to support individuals experiencing rough sleeping in Exeter. This role involves engaging with people both on the streets and within emergency accommodation, including sleeping pods, to help them transition into safer, more stable living situations.You will deliver high-quality, person-centred outreach services as part of a supportive team, making contact with new, repeat, and long-term rough sleepers across the Exeter City Council area.Please note: This role requires flexibility, including working weekends and unsocial hours, and the ability to walk several miles in all weather conditions.Key ResponsibilitiesConduct assertive outreach and visit known rough sleeping hotspotsManage referrals and support individuals accessing emergency accommodationComplete needs assessments, support plans, and risk assessmentsEmpower clients by identifying their strengths and goals, and supporting their personal developmentMaintain accurate and up-to-date client records using the internal case management systemUndertake additional duties in line ..... full job details .....
