South locality of Bradford , with the flexibility to work across the wider Bradford area as service needs require.
About the roleAs a valued Health Care Support Worker, you will be an integral part of our planned care community nursing service, delivering essential care and support to patients in their own home environment. No two days are the same — you will be working closely with patients, families, and colleagues, ensuring that care is safe, effective, and person-centered.
Training and developmentWe are committed to helping you grow and develop in your role. You will receive a comprehensive training package and be supported to build a wide range of clinical skills. This includes wound care, continence care, diabetes management (including insulin administration), enteral feeding support, and medication administration. These are just some of the rewarding and specialist skills you will gain through structured training and ongoing support from our experienced clinicians.

Responsibilities and work patternTo work as part of the Adult Community Nursing Services over a 7 day period. Ensuring high quality health care is delivered to patients in their homes as an integrated part of the mainstream Integrated District Nursing service. To work in collaboration with the wider Primary Health Care team, whilst participating in the assessment of health needs and delivering of care to the practice population. The service is delivered over a seven day rota with a shift range from 8–9pm, 7 days a week.
Development opportunitiesWe have great development opportunities for our Health care support workers supporting apprenticeship programmes to undertake pre registration training to be come a registered nurse or Nursing associate.
EligibilityThis position is not eligible for sponsorship set by the Home Office and the skilled worker visa occupation list. If you are currently a visa holder and your visa status expires, we may be required to terminate your employment in compliance with immigration regulations.
