Health and Wellbeing Practitioner - Part Time - Heathrow IRC Location: London Salary: £16,200 per annum Vacancy Type: Permanent About The Role Are you passionate about supporting people to improve their health, wellbeing and recovery outcomes? The Forward Trust is seeking a compassionate and motivated Health and Wellbeing Practitioner to join our integrated healthcare team at Heathrow IRC on a part-time basis (21 hours per week). You''ll support detainees with substance misuse, low-level mental health needs and wider wellbeing concerns, helping them build resilience and prepare for the next stage of their journey. What you''ll be doing Working as part of a multi-disciplinary team, you will: Complete assessments, risk management plans and recovery plans Manage a caseload and deliver both 1:1 and group interventions Support individuals with substance misuse and mental health needs Develop release, discharge and continuity of care plans Promote wellbeing services and signpost to relevant support Contribute to multi-disciplinary reviews and daily team meetings Maintain accurate case records and reports Provide transitional support to individuals preparing for release or deportation What we''re looking for We''re seeking someone who is: Passionate about health, wellbeing and recovery Confident managing a varied caseload Skilled in building positive and professional relationships Organised, proactive and able to maintain high-quality records Comfortable working within a ..... full job details .....
