Location: Devon (Field-Based)
Regular travel throughout the designated area required.
Salary: £31,000 per annum
Contract: Permanent, Full-Time
Hours: 37 hours per week
  
Role Overview
Are you passionate about helping people overcome barriers, build confidence and create positive, lasting change?
We are recruiting a dedicated and person-centred Health & Wellbeing Advisor (Progression Advisor) to join an innovative employment and wellbeing programme supporting individuals whose health conditions, disabilities, wellbeing challenges or life circumstances have impacted their confidence, independence and participation.
  
As a Health & Wellbeing Advisor (Progression Advisor), you will build trusted relationships with participants, providing tailored coaching and practical support that helps people move forward at a pace that works for them. Rather than focusing solely on employment outcomes, you will support individuals to improve wellbeing, develop healthy routines, build resilience, increase community participation and explore future opportunities, including employment where appropriate.
  
Working as part of a multidisciplinary team alongside Occupational Therapists, Wellbeing Specialists and Employment Professionals, you will act as the participant''s primary point of contact, coordinating support and helping individuals achieve meaningful and sustainable progress.

Key Responsibilities
  
Participant Engagement & Relationship Building
· Build and maintain trusted relationships with participants throughout their journey
· Create a positive, supportive and empowering environment for change
· Deliver person-centred support that reflects individual needs, goals and circumstances
· Maintain regular contact through face-to-face appointments, outreach activity and other engagement methods
· Act as the primary point of contact for participants accessing the service
  
Wellbeing & Progression Support
· Deliver personalised coaching focused on confidence, wellbeing, participation and independence
· Support individuals to identify strengths, aspirations and opportunities for personal development
· Help participants establish healthy routines, self-management strategies and practical coping techniques
· Encourage positive behaviour change and long-term resilience
· Support participants to increase engagement within their local communities
  
Employability & Future Planning
· Provide practical employability support where appropriate, including:
o CV development
o Job search support
o Interview preparation
o Labour market exploration
o Employment planning
· Support participants to explore volunteering, training and learning opportunities
· Develop in-work support plans that promote sustainable employment outcomes
· Encourage progression into suitable education, training or employment opportunities
  
Partnership & Community Engagement
· Facilitate access to community activities, support networks and local services
· Develop relationships with local employers, training providers and partner organisations
· Coordinate referrals to specialist support services when required
· Work collaboratively with Occupational Therapists, Wellbeing Specialists and external stakeholders
· Promote available services within local communities
  
Quality, Compliance & Administration
· Maintain accurate participant records and action plans
· Monitor and review participant progress against agreed goals
· Ensure all activity complies with contractual, safeguarding and organisational requirements
· Maintain accurate case notes and documentation
· Support service performance, quality standards and reporting requirements
  
Essential Criteria
· Full UK driving licence and access to a vehicle
· Ability and willingness to travel extensively across the designated region
· Experience managing a caseload and working independently
· Experience supporting individuals through:
o Coaching
o Mentoring
o Employability support
o Wellbeing services
o Community-based support
o Advice and guidance
· Strong organisational and time-management skills
· Excellent communication and interpersonal skills
· Ability to build trust and rapport with a wide range of people
· Experience creating personalised action plans and support pathways
· Understanding of the impact health, wellbeing and life circumstances can have on progression and participation
· Knowledge of person-centred and strengths-based approaches
· Proficient in Microsoft 365
  
Desirable Criteria
· Experience delivering outreach or community-based services
· Knowledge of local health, wellbeing and community support provision
· Understanding of motivational interviewing techniques
· Understanding of behavioural coaching methodologies
· Experience within:
o Employability
o Welfare-to-Work
o Supported Employment
o Employment Support Programmes
o Community Services
o Health and Wellbeing Services
· Careers Information, Advice and Guidance (CIAG) qualification or experience
  
Additional Information
· Field-based role with regular travel throughout the local area
· Enhanced DBS and compliance checks required
