Community Reablement Assistant

An exciting opportunity has become available for a caring and dedicated Community Reablement Assistant to join a specialist community reablement service supporting individuals in their own homes across Bournemouth.This rewarding role focuses on improving lives and building independence by providing short-term, person-centred support to individuals who need assistance following illness, injury, hospital discharge or a change in circumstances. The aim is to help clients regain confidence, maintain their daily living skills and remain as independent as possible within their own homes and community.As a Community Reablement Assistant, you will provide practical and emotional support to individuals, helping them achieve their personal goals and agreed outcomes.
