Community Mental Health Social Worker

Overview
Dunstable Community Mental Health Team provides compassionate person-centred interventions to people experiencing mental ill-health in the Towns of Dunstable and Leighton Buzzard. The team has access to new facilities at Grove View Health and Care Hub in Central Bedfordshire. The Social Care Lead offers regular reflective practice sessions and supports continuous learning as part of a strong social care presence within ELFT. The Trust provides established ASYE programmes and bespoke training opportunities as part of the yearly appraisal cycle.Responsibilities
Work as part of a multi-disciplinary team to deliver high-quality social care services to adults experiencing mental ill-health.Undertake comprehensive Care Act assessments and reviews, assess eligibility under the Care Act 2014, and lead in safeguarding practices.Promote appropriate health and social care services, commission services as required, and promote Direct Payments in care planning.Maintain accurate records on the service users’ Electronic Patient Record and participate in regular risk assessments to safeguard vulnerable people.Engage in reflective practice, continuing professional development, and supervision (including management and reflective supervision for newly qualified staff).Support newly qualified social workers through the Assessed and Supported Year in Employment (ASYE), with opportunities to train as an ASYE assessor.Undertake duties as an Approved Mental Health Professional (AMHP) or undertake AMHP training when required; participate in regular supervision and reflective practice as a social worker and AMHP.Contribute to discharge planning and inter-agency collaboration with acute inpatient units, Crisis Team, Primary Care services, and the voluntary and private sectors to support recovery journeys.Adhere to Trust policies and procedures and undertake additional training as required.Person specification / Qualifications
Essential: Qualified Social Worker registered with Social Work England.Desirable: Degree level education (if applicable) and relevant post-qualifying/post-graduate training.Post-qualification experience within mental health services is desirable.Experience in AMHP role or Best Interest Assessor (BIA) is desirable; social workers are expected to train and practise as AMHPs.Additional requirements
