The CNS for older people and Frailty will take a senior role within the Care of Older People (COOP) team. They will provide a role which is 80% clinical, 20% quality improvement, service development and management. The post holder will be responsible for coordinating the flow of frail older people through the Whittington hospital and ensure access to appropriate community services. Whittington Hospital, Magdala Ave, London, N19 5NFJob Description and ResponsibilitiesClinical Responsibilities: Review patients screened as frail in the Emergency Department, Ambulatory care and the Acute Admissions wards, according to Frailty Pathway criteria. In conjunction with the Consultant Geriatrician/Frailty MDT provide expert assessment of frail elderly patients on admission to determine the most appropriate pathway of care and direct patients accordingly. Liaise closely with multidisciplinary team members including community colleagues and within the hospital. Ensure good communication to enable safe and timely discharge and appropriate admission. Act as patient and carer advocate. Communicate with patients, carers and relatives to involve them in care planning and resolve concerns. Assess needs for additional services and refer as necessary. Gain informed consent to intervention and treatment. Provide education and instruction to patients, carers, relatives and other health professionals to promote long-term condition management. Manage potentially stressful situations empathetically, including delivering bad news and palliative care discussions. Arrange/attend team/family meetings and act as a resource across Whittington Health on the care of older people. Initiate and implement changes in clinical practice to improve care and outcomes. Develop standards of practice and align with trust protocols. Identify clinical issues and incidents and develop strategies to counter them. Identify and resolve patient-related or service problems and prioritise conflicting needs.Education: Identify education needs in relation to care of older people. Develop training strategies to improve staff understanding, facilitate development of knowledge and skills within older people, frail elderly and dementia care services. Critically appraise evidence-based literature to lead education programmes. Prepare learning materials and promote research-based practice. Provide written information for patients and carers to enhance understanding. Identify personal training and development needs and participate in appraisal and ongoing professional development.Leadership, Management and Service Improvement: Champion patient-centred care for frail older people with timely diagnosis, interventions and multidisciplinary discharge planning. Contribute to strategic planning for the frailty service and nursing contributions to service improvements. Provide visible clinical leadership, act as a multidisciplinary resource, promote patient safety, motivate staff, and drive continuous improvement. Engage patients, families and carers to gather feedback and involve them in service evaluation. Represent the Trust on committees relevant to care of older people. Plan and organise own time to meet service demand. Involve in development of care guidelines and manage line responsibilities.Research and Audit: Ensure audit programmes align with service objectives and undertake audits to support service development.
