Do not miss out on this fantastic opportunity!Responsibilities of this role:Manage a caseload of Looked After Children and Care Leavers, ensuring statutory responsibilities are met and positive outcomes are achieved.Complete assessments, care plans, pathway plans, and reviews in line with legislation and local procedures.Undertake regular visits and direct work with children and young people, ensuring their views and wishes inform decision-making.Support care leavers in their transition to independence, including access to education, employment, training, housing, and health services.Work collaboratively with families, carers, partner agencies, and professionals to safeguard and promote the welfare of children and young people.Attend and contribute to Child Looked After Reviews, pathway planning meetings, and other multi-agency forums.Identify and manage risks, taking appropriate safeguarding action to protect vulnerable children and young people.Maintain accurate, timely, and high-quality case records, reports, and assessments in accordance with statutory requirements and professional standards.Requirements of this role:Must be a Qualified Social Worker, registered with Social Work England.Must have minimum of 3 year''s experience ..... full job details .....
