An exciting opportunity has arisen for an experienced Care Coordinator to join a domiciliary care service in Coventry.

This role would suit an organised and proactive individual with experience coordinating home care services, managing rotas and supporting care teams to ensure clients receive safe, reliable and person-centred care.

You will play a key role in the day-to-day running of the service, ensuring care packages are effectively coordinated, staffing levels are maintained and high standards of care are delivered.

• Coordinate daily domiciliary care schedules, ensuring visits are covered and delivered on time.

• Prepare and maintain staff rotas, managing changes, sickness and emergency cover.

• Match care workers to clients based on needs, availability and continuity of care.

• Liaise with clients, families and care staff to ensure a smooth delivery of services.

• Respond to queries, concerns and changes in clients'' care requirements.

• Support the recruitment, onboarding and allocation of care workers.

• Monitor care delivery and address missed calls or staffing issues promptly.

• Maintain accurate records, care documentation and computerised systems.

• Support care assessments, care plan reviews and risk assessments where required.

• Ensure compliance with company policies, safeguarding procedures and relevant regulations.

• Build positive working relationships with healthcare professionals and other stakeholders.

• Support the Registered Manager with audits, quality assurance and general branch administration.

• Help identify opportunities to improve service delivery and client satisfaction.

• Participate in on-call duties where required.

We''re looking for someone who is:

• Experienced in a Care Coordinator role within domiciliary care.

• Highly organised, with strong planning and time-management skills.

• Confident managing rotas and responding to last-minute changes.

• An excellent communicator who can build relationships with clients, families and care teams.

• Calm under pressure and able to prioritise competing demands.

• Proactive, reliable and solutions-focused.

• Knowledgeable about person-centred care, safeguarding and care sector compliance.

• Comfortable using computerised care management and scheduling systems.

• Previous experience coordinating care within a domiciliary or home care setting.

• Experience producing staff rotas and managing care cover.

• Understanding of care plans, safeguarding and quality standards.

• Strong administration, communication and problem-solving skills.

• Ability to work effectively as part of a busy care team.
