Prior care coordinator experience is a MUST, as it will aid in efficiently managing administrative responsibilities associated with the role. Duties Coordinate and schedule care plans for clients, ensuring timely and appropriate support. Liaise with healthcare professionals, clients, and their families to facilitate effective communication and service delivery. Maintain accurate and up-to-date records of client information, care schedules, and service documentation. Provide administrative support including managing appointments, preparing reports, and handling correspondence. Monitor client progress and adjust care plans in collaboration with relevant stakeholders. Ensure compliance with organisational policies and relevant health and safety regulations. Assist in the onboarding of new clients and staff members as required. Taking on the responsibility of having the on-call and ensuring all calls are answered and actioned in a professional manner.
