Overview

An exciting opportunity has arisen for a suitably qualified and experienced senior mental health practitioner to work closely with the Team Manager and other senior clinicians within HandF Central MINT services in Hammersmith and Fulham. This post is open to nurses, social workers and occupational therapists and is a management post within the team.Remodelled teams and model

Remodelled community teams have been established over the past year following a successful bid for additional resources. In line with the population health model the team now deliver mental health interventions in partnership with primary care colleagues as well as continuing to provide care for those with more complex needs.The Long-Term Plan, together with NHSE’s Community Mental Health Framework, provides a historic opportunity to address gaps in the provision of Mental Health care within communities. Community teams have moved away from siloed, hard-to-reach services towards joined-up care and whole population management approaches, establishing a revitalised purpose and identity for community mental health services.One of the LTP’s key objectives is to develop “new and integrated models of primary and community mental health care [which] will support adults and older adults with severe mental illnesses”.Responsibilities

Lead and monitor the performance and care delivery of the nursing staff in the MINT team; ensuring local professional leadership.Review clinical policies and guidelines relevant to the area and support and implement changes to practice.Liaise with other organisations and professionals to promote communication and good practice; undertake audit activity and supervision to other staff in service area/care group.Demonstrate understanding of psychosocial interventions including psycho-education, wellness recovery action planning and discharge planning.Assess care needs and develop, implement and evaluate programmes of care for a small and defined caseload; maintain associated records.Support the team manager in providing effective leadership within a clinical team delivering a high-quality service within defined resources and geographical area.About the Trust
