Overview
An exciting opportunity for a suitably qualified and experienced senior mental health practitioner to work closely with the Team Manager and other senior clinicians within HandF Central MINT services in Hammersmith and Fulham. The post focuses on leading and developing the nursing staff within the MINT team, delivering care in line with population health and integrated care objectives.
ResponsibilitiesLead and monitor the performance and care delivery of the nursing staff in the MINT team; provide local professional leadership.
Review clinical policies and guidelines relevant to the area and support and implement changes to practice.
Liaise with other organisations and professionals to promote communication and good practice; undertake audits and supervise other staff within the service area.
Apply an understanding of psychosocial interventions, including psycho-education, wellness recovery action plans and discharge planning.
Assess care needs and develop, implement and evaluate programmes of care for a defined caseload; maintain associated records.
Support the team manager in providing effective leadership within a clinical team delivering a high-quality service within the defined geographical area.
Qualifications and experienceSenior mental health practitioner with relevant professional background (nursing, social work or occupational therapy).
Experience in leadership/management within a mental health setting; knowledge of psychosocial interventions and discharge planning.
