Band 5 Staff Nurse - Neonatal Community Outreach Team

University Hospitals Birmingham and Birmingham Women''s and Children''s have developed a joint partnership Neonatal Community Outreach Service (NCOT). NCOT is a team of neonatal nurses providing continuing nursing care to premature babies in their home environment. NCOT operate a 7-day service working from 8am to 6pm, including bank holidays. Successful candidates will be required to work between the Neonatal in-patient services and in the community setting. They will also need to provide cross cover on occasions between the 2 organisations. Early discharge with the Community Service supports the family integrated care approach and enables families to take their baby home at the earliest opportunity whilst continuing to receive nursing support that meets their medical needs. We are seeking a dynamic, motivated nurse that is passionate about delivering neonatal care within the community.Responsibilities
Band 5 NCOT Nurse will be supported by the NCOT Lead nurse, senior nurses and Multi-disciplinary team to provide high quality care to babies covering a range of conditions including oxygen therapy, phototherapy, nasogastric tube (NGT) fed and babies that are less than 1.8kg at discharge or under 35 weeks gestation corrected as well as other medical or nursing needs.Assess the suitability of referrals and ensure eligibility criteria is met. Involved with the discharge process to ensure a seamless, timely transfer from hospital to the home environment and communicate with families to prepare them for the transition home.Risk assess patient needs, environment and care required during each visit.Maintain neonatal clinical skills including blood sampling, NGT insertion and measuring observations; recognise deterioration or improvement and adapt care plans accordingly.Work as autonomous practitioner, solve problems, function in a lone worker capacity, manage time effectively and demonstrate excellent communication skills.Practice in a holistic, non-judgmental, caring and compassionate manner, support social inclusion and respect diversity; work in partnership with MDT, service users, carers and families.Contribute to practice development within the clinical care setting and comply with Trust Policies, Procedures and Guidelines; complete any work-based training associated with the role.Qualifications and Requirements
Recent Neonatal experience within a hospital setting.Clinical knowledge to identify deterioration or improvement of the patient and adapt care plans.Confidence and competence with nursing skills including blood sampling and NGT insertion; ability to measure observations.Ability to work autonomously, solve problems, and manage time; strong communication skills.Commitment to holistic, compassionate care and collaboration with MDT, service users, carers and families.Equality and Inclusion
