Advanced Clinical Practitioner

We are seeking an experienced Advanced Clinical Practitioner (ACP) / Advanced Nurse Practitioner (ANP) to join a GP surgery in Luton or Dunstable. The ideal candidate will have primary care experience, be proficient in SystmOne, and have experience with COPD and asthma reviews. Responsibilities: As an Advanced Clinical Practitioner / Advanced Nurse Practitioner, you will be responsible for: Providing high-quality clinical assessment, diagnosis, treatment and management of patients within a GP surgery. Conducting patient consultations and supporting the management of a wide range of clinical conditions. Carrying out COPD and asthma reviews and supporting ongoing chronic disease management. Working autonomously within your scope of practice while ensuring safe and effective patient care. Using SystmOne to accurately document consultations, clinical assessments, treatment plans and patient outcomes. Working collaboratively with GPs, nurses and other healthcare professionals to provide integrated patient care. Identifying when patients require further investigation, referral or additional clinical support. Maintaining high standards of clinical governance, confidentiality and professional practice. Contributing to the effective day-to-day delivery of clinical services within the GP surgery.
