Advanced Clinical Practitioner
We are seeking an experienced Advanced Clinical Practitioner (ACP) / Advanced Nurse Practitioner (ANP) to join a GP surgery in Luton or Dunstable. The ideal candidate will have primary care experience, be proficient in SystmOne, and have experience with COPD and asthma reviews.
Responsibilities:
As an Advanced Clinical Practitioner / Advanced Nurse Practitioner, you will be responsible for:
- Providing high-quality clinical assessment, diagnosis, treatment and management of patients within a GP surgery.
- Conducting patient consultations and supporting the management of a wide range of clinical conditions.
- Carrying out COPD and asthma reviews and supporting ongoing chronic disease management.
- Working autonomously within your scope of practice while ensuring safe and effective patient care.
- Using SystmOne to accurately document consultations, clinical assessments, treatment plans and patient outcomes.
- Working collaboratively with GPs, nurses and other healthcare professionals to provide integrated patient care.
- Identifying when patients require further investigation, referral or additional clinical support.
- Maintaining high standards of clinical governance, confidentiality and professional practice.
- Contributing to the effective day-to-day delivery of clinical services within the GP surgery.
Requirements;
The successful Advanced Clinical Practitioner / Advanced Nurse Practitioner will have:
- Previous experience working as an ANP, ACP or Advanced Clinical Practitioner, ideally within a GP surgery or primary care setting.
- Relevant advanced clinical practice qualifications, with a Master's in Advanced Clinical Practice ideally held.
- Experience undertaking clinical assessments and managing patients independently.
- Experience conducting COPD and asthma reviews and supporting patients with long-term conditions.
- Strong working knowledge and proficiency in SystmOne.
- Excellent clinical decision-making, assessment and communication skills.
- The ability to work independently while also working effectively as part of a multidisciplinary team.
- A patient-focused approach with strong attention to detail and a commitment to delivering safe, high-quality clinical care.
- Availability for 10am–5pm shifts, ideally on Tuesdays and Fridays, although flexibility can be considered.
- Ideally, the ability to start as soon as possible.
