The post holder will assess needs, manage risk, promote independence, and work collaboratively with individuals, families, carers, and partner agencies to improve wellbeing and achieve positive outcomes.Key ResponsibilitiesUndertake comprehensive social care and mental health assessments in line with relevant legislation and professional standards.Develop, implement, and review person-centred care and support plans.Build effective relationships with young adults to empower them in making informed decisions about their lives and wellbeing.Safeguard vulnerable adults and manage risks through appropriate interventions and multi-agency working.Support young adults transitioning from children''s services into adult mental health services.Work collaboratively with health professionals, education providers, housing services, employers, and community organisations.Provide information, advice, advocacy, and signposting to relevant support services.Maintain accurate, timely, and detailed case records using approved systems.Participate in multidisciplinary meetings, case conferences, and safeguarding investigations.Support individuals in accessing education, employment, training, and community ..... full job details .....
