Young People Support Worker

Nocturnal Recruitment Solutions are looking for a Young People Support Worker support worker who can work with young people in a supported living service. The work is based within North/North West London, and the ideal worker will have an extensive background dealing with vulnerable 16-21 year olds who have diverse needs such as challenging behaviour, mental health and emotional needs. As a Young People Support Worker you will need to Have a Level 3 Diploma in Health and Social Care or Residential Childcare or Relevant Bachelors Degree (or Higher) e.g Social Work, Youth Studies. Roles and responsibilities: Assess practical and emotional needs, developing care plans that outline the required care and support in a person-centred approach. Ensure risk assessments are actioned as necessary, in line with policies and procedures. Be responsible for the delivery of services to children and young people in the home, in such a way that safeguards and promotes their welfare. Collate data statistics and undertake administrative duties as well as completing daily logs. Work in partnership with other professionals to ensure that the individual gets the support they need. Accompany residents in meetings with other agencies and health professionals as well as liaising with other support workers, social workers, YOs Work with residents families to help them to support the young person in the most effective possible way.
