We''re looking for an experienced Transition Officer to support learners with EHCPs, SEND and additional learning needs as they transition into college. You''ll work closely with students, families, schools, SENCOs and Local Authorities, coordinating transition and admissions, supporting EHCP consultations, completing pre-entry assessments and ensuring learners have the right support in place.
