As Team Manager for our Hospital Liaison Team, you''ll lead a skilled team helping people leave hospital safely and get the right support in place for the next stage of their lives. Working closely with hospitals, health services and community partners, you''ll make sure people experience smooth, timely transitions from hospital and receive person-centred support that helps them stay as independent as possible.
