Duties will include:- Effectively manage risks of the people we support through ensuring team members understand what is important to the individual(s), involving their families, wherever possible, and ensuring decisions are accurately recorded.- Work in unison with families and peoples circle of support.- Develop and maintain strong partnerships with families, carers, advocates, social workers, healthcare professionals and other stakeholders, recognising the vital role they play in achieving positive outcomes and ensuring a coordinated approach to support.- Lead and manage the team to support personalised rota planning (inclusive of day, night and overnight activity) ensuring shifts are proactively designed to maximise opportunities for people in response to their needs, interests and goals.- Manage overall key work with individual(s) developing their support plan through the ..... full job details .....
