      <description>Location: Sutton | Salary: &amp;pound;32.00/hour | Type: Temporary | Eden Brown Synergy are currently looking for an experienced Qualified Social Worker to join the Hospital Pathways Team at Sutton Council.    The role involves supporting adults through hospital pathways and discharge planning, alongside safeguarding, Care Act assessments, mental capacity assessments, continuing healthcare assessments, and complex case management.     Duties and Responsibilities     Manage an allocated caseload within the Hospital Pathways Team, ensuring effective and timely case management.  * Triage allocated Adult Safeguarding cases and undertake safeguarding enquiry planning and coordination.  * Arrange and participate in multi-disciplinary meetings with health and social care professionals.  * Complete comprehensive risk assessments across community settings, nursing homes, and care homes.  Undertake Mental Capacity Assessments and facilitate Best Interests Meetings relating to long-term care and support arrangements.
