The role involves assessing needs, promoting independence, and supporting individuals to remain safe and well in the community. You will use an asset-based approach and work closely with families, carers, and multidisciplinary teams to develop outcome-focused support plans. The post requires strong professional judgment, risk management skills, and the ability to support individuals through changing, complex circumstances. Key Responsibilities: Complete Care Act assessments and identify eligible needs for adults and carers. Develop, review, and monitor person-centred support plans promoting choice, control, and independence. Encourage involvement and ownership of care planning, including clear explanation of Direct Payments. Support individuals to retain skills, confidence, and prevent or delay deterioration. Work with families to facilitate contingency planning. Consider NHS Continuing Healthcare as part of assessment work. Support the ..... full job details .....
