Supporting People on Probation Navigator Location: Cambridgeshire Salary: £26,000 per annum Vacancy Type: Permanent About The Role We are looking for a dedicated and client-focused Health and Wellbeing Practitioner to join our team, supporting adult men and young men within the Criminal Justice System. Working alongside a team of practitioners, subcontractors and partner agencies, you will provide personalised Personal Well Being Services designed to support individuals to make positive changes and improve their wellbeing. The support you provide will be tailored to each individual''s needs and may cover: Family and Significant Others Lifestyle and Associates Emotional Wellbeing Social Inclusion What You''ll Be Doing As a Health and Wellbeing Practitioner, you will: Manage an allocated caseload, ensuring individuals receive appropriate and consistent support. Build positive, professional and trusting relationships with service users. Assess individual needs and develop appropriate care and support plans. Provide advice, guidance and practical support based on individual circumstances. Deliver structured 1:1 sessions and group-work programmes across the Personal Well Being Services. Support individuals to develop positive coping strategies, improve their emotional wellbeing and make sustainable lifestyle changes.
