Providing high-quality, person-centred support to young people with complex care needs within community-based residential settings. Supporting teenagers and young adults living in single occupancy placements with staffing ratios ranging from 2:1 to 4:1. Promoting independence, wellbeing, positive outcomes and life skills development. Building and maintaining positive relationships with young people, families and external professionals. Following and implementing support plans, risk assessments and positive behaviour support strategies. Supporting young people through challenging situations and periods of crisis using de-escalation techniques. Carrying out approved physical intervention practices where necessary and appropriate. Maintaining accurate records, incident reports and daily documentation. Acting as a positive role model and supporting less experienced team members.
