You will provide person centred support that empowers individuals to build independence, improve wellbeing, develop life skills and access the services they need. Key Responsibilities include Work closely with clients to assess their needs and create tailored support plans with clear goals and outcomes. Carry out risk assessments, safety planning and risk management plans. Support vulnerable adults and children, ensuring safeguarding concerns are recognised, reported and managed appropriately. Help clients access external services including housing, healthcare, legal support, benefits, education and employment. Encourage independence, resilience, positive parenting and community involvement.
