Support Worker

Days and waking nights available Dedicated and compassionate Support Worker responsible for providing high-quality care and support to individuals with physical disabilities, learning disabilities, mental health conditions, autism, or other support needs. Key Responsibilities Deliver person-centred care and support in accordance with individual care plans. Assist service users with daily living activities, including personal care, dressing, bathing, meal preparation, and household tasks. Support individuals in developing life skills and maintaining independence. Accompany clients to appointments, educational activities, community events, and social engagements. Administer medication and maintain accurate medication records in line with organisational policies. Monitor and report changes in clients'' physical, emotional, and mental wellbeing. Maintain accurate documentation, progress notes, and incident reports. Promote safeguarding, equality, diversity, and inclusion at all times. Provide emotional support and encouragement to service users and their families. Work collaboratively with healthcare professionals, social workers, and multidisciplinary teams. Ensure compliance with health and safety regulations, confidentiality requirements, and organisational procedures.
