Support Worker

This role is to work shifts and will require occasional weekend work. Sounds great, what will I be doing? In this rewarding role, you will work closely with service users to deliver person-centred, strengths-based support that promotes recovery, independence, and wellbeing. You will engage individuals in comprehensive assessments, support planning, and safety planning, ensuring that all plans are user-led and tailored to their unique needs, aspirations, and goals. Working collaboratively with the Service Manager/Team Manager, you will assess referrals and develop, review, and monitor support, safety, and move-on plans. You will build strong partnerships with external agencies, families, carers, and other key supporters to ensure coordinated and effective support. The role also involves helping service users access healthcare, education, training, employment, and other opportunities that support their recovery journey, while encouraging participation in social and skills-development activities that promote inclusion and confidence.
