As a Support Worker, you will provide person-centred support to service users by carrying out comprehensive assessments, support planning, and safety planning that promote recovery, independence, and individual wellbeing. Working closely with the Service Manager, Team Manager, external agencies, families, carers, and other key supporters, you will help develop and review support, risk, and move-on plans tailored to each individual''s needs.
